📍 Charlotte, NC
💰 Salary Range: $60,000 – $70,000/year (based on experience)
🕘 Full-Time | Monday – Friday | 8-Hour Shifts

In North Carolina, our Assertive Community Treatment Team (ACTT) program provides high-intensity, community-based behavioral health services—bringing treatment directly to individuals who face barriers accessing care.
About the ACTT Program
The ACTT model serves as a “hospital without walls,” providing integrated, wraparound support to individuals with severe and persistent mental illness. Our multidisciplinary team addresses the full spectrum of client needs—mental health, substance use, vocational, medical, and housing—through a person-centered, recovery-oriented approach.
ACTT nurses play a vital role in supporting medication management, health education, and coordination of medical care, ensuring every client receives consistent, comprehensive support in the community.

Key Responsibilities
-
Coordinate client care and communicate with healthcare providers to support holistic treatment
-
Monitor medication compliance and administer prescribed IM injections
-
Manage pharmaceutical inventory, supplies, and medication refills
-
Record vital signs and assist physicians during client appointments
-
Conduct AIMS assessments upon admission and every six months thereafter
-
Provide medication education to staff, clients, and families
-
Transport clients as needed to access community resources and medical appointments
-
Travel within the community to deliver direct nursing and support services
-
Participate in crisis response rotation, including after-hours assessments, hospital diversion, and commitment procedures

Perform other duties as assigned
Qualifications
✅ Required:

Current Registered Nurse (RN) license in North Carolina

Valid Driver’s License

Ability to work both independently and collaboratively in a community-based setting
