Job Title: Case Coordinator
Schedule: M-F, 8 AM - 4 PM
Summary:
The Case Coordinator assists individuals in achieving physical and mental wellness through the planning of health and social services. Responsibilities include advocacy, assessment, planning, and education. The Case Coordinator, along with the care team, ensures individuals are established with services in a coordinated, effective, and efficient manner. The role involves offering support and monitoring an individual's progress throughout treatment.
Essential Duties and Responsibilities:
- Coordinate client engagement through appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
- Maintain communication with referral sources and outside providers through consistent, professional interactions, assessing needs, and documenting contacts during admission and throughout treatment.
- Coordinate transportation needs for clients.
- Provide individual case coordination sessions.
- Assist in developing aftercare plans, including referrals and addressing outside provider needs.
- Support clinical services by facilitating groups and encouraging client engagement in the program.
- Collaborate with the treatment team, maintaining professional communication with counselors.
- Document client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines.
- Work with the client navigation team for aftercare needs.
- Provide onsite support for Enhanced Outpatient Sessions (EOS) appointments between clients and the Client Navigation team.
- Participate in mandatory corporate training, program training, and professional development.
- Perform other duties as assigned.
Education, Licensure, & Experience:
- High School Diploma or equivalent required.
- 1 year of experience in a drug and alcohol or mental health setting preferred.
- Driver’s License preferred.
- CPR/BLS certification preferred.
Employment Information:
Due to the nature of behavioral healthcare work and our commitment to ensuring client safety, candidates selected for hire must successfully pass a criminal background check, drug screening, and any required state-mandated clearances. Employment is contingent upon meeting all screening requirements in accordance with federal, state, and accrediting-body regulations.
