How can I request records?

Simply complete the request form below and attach any supporting documents such as releases of information or court orders.  You may also request records via fax or mail.  Please call us at (814) 940-0407 or (888) 694-9996 extension 1214 for more information. Your confidentiality is our priority. Your privacy is important. Pyramid Healthcare keeps your health records confidential, however, they are readily available when you need to share information.

Who owns my health records and is there a fee for obtaining them?

The information contained in your health records belongs to you, and you are entitled to access that information. If you wish to receive a paper copy of your records for your own use, you may be charged a fee for processing the request. There is no fee for transferring your health records directly to another healthcare provider.

Can health records be shared without consent?

Yes. The law allows for exceptions and typically limits the amount of information that can be released under special circumstances. Some examples would be in response to a medical emergency or under a court order. Otherwise, a valid release of information must be signed in order to share health records.

Can I request changes to my health record?

Every client can request a change to their health record if they believe that there is incorrect or incomplete information. Please request any changes by sending an email to [email protected] or a letter to Health Records Department, PO Box 967, Duncansville, PA 16635. A member of the Health Records Department will contact you within 60 days of the request to discuss. Please note, we will consult the program you attended prior making changes to your record. Pyramid Healthcare can also deny the request if we deem your record correct and complete.
