Case Coordinator (Full-Time)

Summary:

Case Coordinators are responsible for managing and coordinating care for clients and families in residential treatment settings. This role includes providing individual and family case management services, facilitating group education and skill development, and collaborating with the clinical team on admission, aftercare/discharge planning, and referral and external provider coordination and support.

Shift options are Monday - Friday, 8:00am - 4:00pm, 9:00am - 5:00pm, or 10:00am - 6:00pm.

Compensation is $17.50/hr before shift differentials.

 

Responsibilities:

  • Coordinate client engagement through appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
  • Maintain consistent, professional communication with referral sources and outside providers, assess needs, and document contact from admission through the duration of treatment.
  • Provide case coordination services, including arranging transportation and conducting individual case coordination sessions.
  • Assist in developing aftercare plans, including referral and aftercare provider contact, and addressing outside provider needs.
  • Support clinical services by assisting with groups and supporting clients weekly to enhance engagement in the program.
  • Collaborate with the treatment team and maintain professional communication with counselors and team members.
  • Document client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines.
  • Collaborate with the client navigation team for aftercare needs and offer onsite support for Enhanced Outpatient Sessions (EOS) appointments.
  • Participate in mandatory corporate training, program training, and professional development.
  • Perform other duties as assigned.

Qualifications:

  • High School Diploma or equivalent required.
  • 1 year of experience in a drug and alcohol or mental health setting preferred.
  • Driver's License preferred.
  • CPR/BLS certification preferred.
