Summary

Join us in making a difference! As a Client Navigator, you will play a vital role in facilitating clinical services for our program, adhering to facility standards and embracing the clinical philosophy of our organization. You will support clients within Pyramid Healthcare, ensuring they experience a seamless transition in their continuum of care.

Essential Duties and Responsibilities:

  • Evaluate the behavioral, emotional, cognitive, spiritual, and social needs of clients and their families. Collaborate with the treatment team regularly to develop and monitor progress towards treatment goals, all while adhering to clinical policy and accreditation guidelines.
  • Facilitate individual sessions to create tailored treatment plans.
  • Oversee all clinical services for clients, including individual and family sessions, therapeutic groups, and coordination with referral sources and outpatient providers as needed.
  • Maintain clinical records for clients with precise documentation of clinical services and client engagement in ECR, following program policy.
  • Provide timely documentation and support for utilization review, actively participating in the UR process as per program policy.
  • Engage in aftercare plan development and contribute to the outcome data process.
  • Deliver case management services for clients and families, coordinating transportation needs, providing individual and family case management sessions, assisting in aftercare plan development, and addressing external provider needs. Conduct Level of Care Assessments as required.
  • Complete all mandatory trainings as designated by the company and accreditation/licensing entities.
  • Take on other duties as assigned.

Required Qualifications

Education, Licensure, & Experience

  • Preferred Bachelor’s Degree in human services or a related field, along with 1-2 years of experience in the substance abuse and mental health field. A high school diploma or equivalent will be considered with 1-2 years of experience in similar work.

Physical Demands

While performing the duties of this job, employees frequently need to talk or hear. They regularly sit, stand, use their hands to finger, handle, or feel, and reach with hands and arms. Walking, stooping, twisting, kneeling, or crouching is seldom required, and employees may occasionally lift and/or move up to 10 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Work Environment

This position is onsite, where the noise level at facilities and offices is typically moderately quiet and indoors.
